Thought-Provoking Questions

Often, when I am booked for lectures or radio and television interviews, I am provided questions in advance to help me prepare. Some of the questions are provocative, the kind that make me stop and think. So much so, I am sharing a few here with my replies.

1. What are three principles you live by?
a) “Identify the fear and then go there.” Usually I’m afraid because it is something I have never done before, and so I figure I will at least learn something new. I always ask myself: What is the worst-case scenario? If it’s one I can deal with, I go for it.
b) “Not everything worth doing is worth doing well.” Being stuck striving for perfection will not allow you to move on. For me, this pertains to writing. It is never finished and can always be improved upon. But at some point, it must be abandoned. Of course, this does not apply to everything; be sure to use discernment.
c) “Maintain an open mind.” I try to not be judgmental or assume that I know best. I listen and attempt to understand other points of view, seeing them in the contexts of culture and personality. When presented with a new opportunity, I ask myself: Will I learn something?

2. Do you believe nature or nurture has more impact?
People are born with specific tendencies; some children are shy, some are outgoing. This is a natural component of our genes, but then life experiences also impact our brains. Successes can reinforce behavior while failures discourage future attempts. So it is both. First it is nature, then nurture. Both have an equal influence.

3. What do you wish you knew then that you know now?
I wish I had not cared so much about what other people thought of me. As women, we try to please others more than necessary. Speak the truth as often as possible and be willing to be disliked, but respected.

4. What did you wish to be when you were a child?
My childhood dream was to be a poet. I won a poetry contest for a poem I wrote when I was eight. This early success inspired me to continue writing poems, which I still do today.

5. What role does luck play in success?
Obviously, luck plays a part–you have to be at the right place, at the right time, with the right people. However, success is also “opportunity meeting preparation.” I am always scanning my environment to look for ways to make a contribution and a difference.

6. What are the barriers to female leadership?
“MEN!” There is still gender discrimination, both conscious and unconscious, even by well-meaning men. Our culture continues to be dominated by male values, and, in the majority of cases, even when shown the same resume with a male or female name, men are chosen over women for both promotions and as new hires.

7. What was your biggest challenge?
a) Acceptance. As the only woman on the faculty of a business school, the first woman in a Rotary Club, the only female consultant in all-male organizations, I faced issues of acceptance. I did not try to blend in, as I knew this was not possible, so I stood out by wearing red suits, sitting on the arms of chairs to make me look taller (I am barely 5 feet tall).
b) A close second was being ignored–not only feeling invisible, but, also, not heard, and (when earned) not rewarded.

8. What changes in the workplace would you like to see?
I would like to see flexible working hours as a norm at work. This would permit time off not only for child-related needs, but, also, for the increasing needs of aging parents. Men must be provided with the same opportunity to take an equal share of these responsibilities without negative consequences in the workplace. What do you attribute your success to?
The willingness to ask the first question, using an assertive voice, looking others in the eye while conversing, giving credit when credit is due, looking and sounding confident (even when I do not feel that way), being well-prepared by knowing the agenda and relevant facts, and lastly, believing in myself.

9. What are the three rules you live by?
a) First of all, have fun. b) Do no harm. c) Be as helpful to as many people as possible.
I am urging you, my readers, to do the same. Think of each question as it relates to you, your values, philosophies, experiences, your life overall. Take time to answer these questions honestly and you might gain new insights–perhaps discover some unconscious motivations.

Trump’s Doctor Has An Amazing Explanation For That Bizarre Medical Report

Donald Trump’s personal physician told NBC News that he wrote a now-infamous medical report about the candidate in just five minutes.

Dr. Harold Bornstein, a gastroenterologist who has been Trump’s doctor for more than three decades, released a brief note last December declaring the reality TV star would be “the healthiest individual ever elected to the presidency.” The four-paragraph letter, which billed Trump’s health as “astonishingly excellent,” was far less detailed than medical reports traditionally released by presidential candidates. 

In an interview with NBC airing Friday, Bornstein said he spent “five minutes” on the letter while a car sent by Trump waited for him outside his office in Manhattan. 

“I get rushed and I get anxious when I get rushed, so I try to get four or five lines done as fast as possible,” Bornstein explained. “In a rush, I think some of those words didn’t come out exactly the way they were meant.”

The doctor stood by his assessment of the reality TV star’s wellness.

“His health is excellent, particularly his mental health,” he said. 

He also defended the claim that Trump would be the healthiest president in history.

“I like that sentence to be quite honest with you, and all the rest of them are either sick or dead,” he said.

Bornstein and his bizarre note have come under scrutiny again as Trump and his campaign surrogates have floated conspiracy theories about Hillary Clinton’s health. MSNBC’s Rachel Maddow pressed Trump campaign manager Kellyane Conway on the issue during an interview Thursday. 

“If he was elected, Donald Trump would be the oldest person to ever be sworn in as president,” Maddow said. “Whether or not he’s going to try to make Hillary Clinton’s health the issue, doesn’t he owe it to the American people to release an actual medical report, a more credible, more complete statement?”

Conway said “perhaps” he should release more information.

Deadly Louisiana Floods Overwhelm Emergency Response Efforts

Deadly flooding in Louisiana is straining state emergency response efforts as officials and residents contend with floodwaters reaching areas that have historically been safe from natural disaster. 

The deluge that started with heavy rainfall Friday has left at least six people dead. First responders have rescued more than 20,000 people, officials said. 

“I was calling 911, but I couldn’t get them on the phone,” Baker resident Margaret Teague told NOLA.com on Sunday. Teague, whose home in an eastern suburb of Baton Rouge was partially destroyed, lost another home to Hurricane Katrina 11 years ago. 

AT&T service was down for an unknown number of residents in the metro Baton Rouge region after a switching tower that directs cellular phone traffic flooded, further complicating rescue and evacuation efforts. 

Teague told NOLA.com her family was among residents bounced from one makeshift shelter to the next before finding one where they could remain.

Roughly 120 people from a Baton Rouge Salvation Army shelter had to evacuate after the headquarters flooded with more than six feet of water, The Associated Press reported. Other families had to sleep on the floors of shelters that had run out of beds, state Department of Children and Family Services head Marketa Walters told Reuters. 

Gov. John Bel Edwards (R), who on Friday declared a state of emergency, was forced to evacuate the governor’s mansion in Shreveport with his family. 

Mike Steele, communications director for the Louisiana Governor’s Office of Homeland Security and Emergency Preparedness, told weather.com on Sunday that rescue efforts were “all hands on deck.”

Steele said motorists were stranded on Interstate 12, and parts of the highway remained closed. At least 2,700 homes from Baton Rouge to the eastern rural Tangipahoa Parish have flooded, he added. 

President Barack Obama on Sunday issued a disaster declaration that makes affected parishes eligible for federal relief. 

According to the Advocate

The federal declaration that Louisiana suffered a major disaster frees the federal government to pay about 75 percent of the state’s expenditures in dealing with the emergency. Higher matches, up to 90 percent federal dollars to 10 percent of state spending, are available depending on how expensive the disaster eventually becomes. 

Just five months earlier, Louisiana was slammed by springtime flooding. 

The cost of the latest damage hasn’t been estimated. State Trooper Bryan Lee told The Huffington Post Monday that financial issues aren’t a priority for now.

“One of the challenges we’re facing is the interstate system being closed, and getting resources and rescuing people that need to be rescued,” Lee said. “As far as financial issues, that’s something we’re not concerned with right now.” 

Lee estimated that nearly 4,000 residents were being housed at metro Baton Rouge-area shelters, including 3,000 people at the Celtic Studios ― a movie studio.

Jay Dardenne, Louisiana’s commissioner of administration, told the Advocate the state is spending whatever necessary responding to the floods for search, rescue, shelter and evacuation. 

The Federal Emergency Management Agency, which handles disaster- assistance, has been under pressure from Congress to trim how much the federal government gives to states for disaster response.  

Louisiana has frequent natural disasters from floods and hurricanes, but is poorer than states like California, which faces wildfires. That financial situation complicates Louisiana’s ability to recover from disasters and invest in prevention efforts.

Jesse Pines, a professor of health policy and management at George Washington University, wrote in a 2014 report that funding for disaster planning and recovery tends to be “lumpy.”   

“A disaster happens and there’s a big flood of resources to take care of the disaster,” Pines said. “And over time as different disasters hit and there are different funding priorities, there’s less money being invested in prevention.”

Aetna Drops Obamacare In Most States

Aetna Inc, the No. 3 U.S. health insurer, on Monday said that due to persistent financial losses on Obamacare plans, it will sell individual insurance on the government-run online marketplaces in only four states next year, down from the current 15 states.

Aetna’s decision follows similar moves from UnitedHealth Group Inc. and Humana Inc., which have cited similar concerns about financial losses on these exchanges created under President Barack Obama’s national healthcare reform law.

Aetna is also trying to buy Humana and is currently fighting a U.S. government lawsuit aimed at blocking the $ 34 billion deal.

Aetna, which earlier this year said it was too soon to give up on the exchanges despite its challenges, this month signaled it was reconsidering. On Aug. 2, the company said it would not expand in 2017 and would review all its individual business.

Many insurers last year had said they expected to profit on the exchanges, but now say more exchange rules must be changed for it to be sustainable. Only about 11 million people have signed up through the exchanges, about half as many as expected.

Larry Levitt, healthcare economist at the Kaiser Family Foundation, said that if enrollment stagnates, small technical fixes likely will not be enough to bring big insurers back. “More healthy people need to be encouraged to sign up,” Levitt said by email.

Aetna said that the current risk adjustment system, in which the government balances out the costs of sicker than typical populations through payments to insurers, is not adequate.

Aetna said that it had a second-quarter pretax loss of $ 200 million on the individual business and has lost $ 430 million since the plans first went on sale in 2014.

Kevin Counihan, Chief Executive of the U.S. government-run individual marketplace, said that the risks of the customer pool is improving.

“It’s no surprise that companies are adapting at different rates to a market where they compete for business on cost and quality rather than by denying coverage to people with preexisting conditions,” Counihan said in a statement.

Aetna on Monday said it would continue to sell plans on the exchanges in Delaware, Iowa, Nebraska and Virginia in 2017. The plans, which are eligible for income-based government subsidies, will be sold in 242 counties, down from 778 counties this year. States it is exiting include Florida and Ohio.

It will continue to sell individual plans that meet the law’s requirements – such as not denying coverage to people with pre-existing conditions – outside of the exchanges where it does now, the company said.

Madam President Or Madman President?

There are less than 100 days until the Presidential election, and voters will have to choose between Hillary Clinton, a former first lady, two-term US Senator, and Secretary of State or Donald J. Trump, a self-glorifying “businessman,” reality television personality, and prior beauty contest promoter. All things considered, I find the title “Madam President” to be preferable over “Madman President.”

Every day there appears a new controversy involving Trump. He is a toxic mix of ignorance and arrogance never before seen in our presidential politics. He has defamed and threatened minorities, advocated war crimes, denigrated war heroes, degraded women, mocked the handicapped, and disparaged the parents of a fallen soldier who sacrificed his life to save his comrades. We are numb to the daily barrage of Trumpisms, and none of his handlers seem to be able to control him.

Trump’s “beliefs” are an ever-changing kaleidoscope, and at the end of the day all he holds sacred is himself. The terrible Trump truth is that the would-be emperor not only has no clothes– he has no character.

He is a serial name caller with the wit and charm of a school-yard bully– hurling such monikers as “Hillary ‘Rotten’ Clinton,” “Crooked Hillary,” “Crazy Bernie” [Sanders],” “Little Michael Bloomberg,” “Corrupt [Tim] Kaine,” “Pocahontas” and “Goofy” [Elizabeth Warren], “Liddle Marco” [Rubio], “Lyin’ Ted” [Cruz], “Low Energy Jeb” [Bush]. He labels other who would dare to challenge his authority “losers,” “lightweights,” and “dummies.” And women who disagree with him are often called “pigs,” “dogs,” and “bimbos.”

I have one name for him, “Delusional Donald.” He has movies playing is his head that no one else has seen, such as thousands and thousands of people cheering on 9/11 from rooftops in New Jersey– or an Iranian propaganda video showing $ 400 million cash paid by the US being unloaded from an airplane (a claim he later recanted). And yet he calls Hillary “unstable.”

What is around the corner for Trump? The debates present a dilemma for the Delusional Donald. I am sure that part of him wants to weasel out of a face-to-face showdown with her. He has already been whining that two of the three debates conflict with NFL games. Hillary is scary to him. She has an unfair advantage over Trump–she actually knows “stuff.” On the other hand, he has done poorly in organization and fund raising as compared to her– she can outspend the “very rich” Trump in campaign advertising and other venues. And she is beating him badly in the polls, which he now has ceased citing. His last chance to turn the tide may be the debates.

And if it becomes clear to him that he can’t win the election, I predict he will simply quit– blaming the media, the GOP, the Democrats, and his most recent scapegoat, the “rigged system.” He would rather save face than to suffer a humiliating defeat in November.

Trump really could end the United States of America as we know it. He has called for the rounding up and deporting of 11 million undocumented people. Do you think when his storm-troopers knock on the doors of these families, they will go peacefully into the night with his enforcers?

And his wall? What utter nonsense. The US/Mexican border spans nearly 2000 miles. How high is Trump’s fantasy fence going to be? Can anyone make a taller step ladder? He once said they may name the wall after him. I wonder who they will name the tunnels after. And how deep must it be to avoid such tunneling? How do you acquire the land and easements from adjacent property owners to get equipment and supplies in for this fool’s folly? But don’t forget the baloney that Trump will get Mexico to pay for it. I wonder if they will also pay any delinquent taxes he may owe after his alleged IRS audits.

He has advocated the banning of Muslims from entering the country. He has called for torture and worse for suspected terrorists, and said we need to “take out” their families. I am not sure how far he would extend this– are spouses, children, parents, siblings, aunts, uncles and cousins all on the hit list? And I have a hunch that anyone who challenges or opposing him will become a “suspected terrorist” and subject to the wrath of the Donald.

There are likely only two options for the CIA or the military when presented with orders to commit illegal acts or war crimes, both of which are doomsday scenarios:
1) If they obey the orders, we will become a fascist nation, or
2) If they disobey the orders, there is a real chance of a military coup.

Trump is a bully, a braggart, and a bigot– unfit for any political office and unworthy of public trust. He is a fast-talking con man better suited to run a jam-auction on a carnival midway. Every loyal American needs to reject Trump. A number of GOP leaders and donors have put their country above party affiliation and done just that, refusing to endorse or donate to him, with some admitting Hillary would be a better choice. Just today 50 patriotic Republican senior security experts, many former cabinet members and top aides, signed an unprecedented open letter to the New York Times disavowing Trump and encouraging us not to vote for him. This should give cover to others who want to jump off this unstable bandwagon.

Those who both condemn and endorse him are the epitome of hypocrisy. The Republican Party would be better off to repudiate Trump and take their loss on the Presidency while trying to salvage the down-ticket candidates. Four years from now they can try to recapture the Presidency with a credible candidate.

Having a two-party system has been of great benefit to this country, and there are intelligent people of goodwill in both parties. Sadly, Trump is not one of them. Of all the Trump bankruptcies, the most disturbing is his moral one.

I am becoming ever more aware of my “perishability.” When we are children, we worry that our parents will die. As we get older, we worry that our spouse will die. But when is it that we start wondering about our own departure from this world? For me, it started when my first parent died, but it really, truly, hit me when my second parent died. And I was in my fifties already. Nonetheless, having them around, regardless of how old or financially poorer they were, gave a feeling of security, of protection. With them gone, I felt alone and realized: I am the next in line.

This feeling of being the next one to go is reinforced by several factors. When you are working, you are too busy, struggling and handling life to have time to think about death. Retired, you have ample time to think about it and to feel the pain of old age. You are reminded of it every morning when you look at all the pills you are supposed to take. You are reminded of how old you are when you need a new hip or kidney because your body has aged to the point that its parts need replacement.

If you still do not feel that the sun of your life is setting, look at old pictures. How many people in them are still alive? When you open your contact list, how many have passed away? How many of your friends have you accompanied on their last trip to the cemetery?

As you become aware that this is it, life is coming to an end, you ask yourself what life is all about. What did I do all my life? What was the purpose of me living?

If I focus on what life WAS about, I think the answer for me is clear: It was a long list of problem solving. There was no day I did not have to solve some problem. Some were small and insignificant, some I thought were critical, but, in retrospect, what I thought was critical then I now find was not worth the time and energy I put into it.

So is that’s what life is about: Solving problems in chasing the rainbow; Trying to realize some vision or mission we developed for ourselves, whether it is to be a rich man or woman, or to save the world?

As I look at what my life was, I realize I was building a sand castle. After I am gone my work will be eroded and washed away by the realities of life’s demands. People will have their interpretation of what is needed and redo my castle to reflect their needs or values.

So is this it? Life is building sand castles?

Not so. Not at all.

When was I really alive? Not when I was solving problems, no matter how successfully. All my achievements and happiness were short lived. There was always another higher mountain to climb. I spent all my life climbing mountains that existed only in my head.

That is what life was, but what SHOULD life have been, I wonder? When was I truly alive?

When I loved. Those are the minutes that really count and I am not referring to romantic love. Yes, it has its moments of happiness, and falling in love, at least in my life, was pain and mostly belongs to the problem-solving realm. The love I am referring to is doing something of value for others, something that really mattered to them, when I gave of myself unconditionally, with all my heart. When I added value to people’s lives. These experiences gave me energy, prolonged my life. Fulfilled me. I got more than what I gave.

Life is spent in the head solving problems, but should be spent in the heart giving of ourselves to solve other people problems. Prolonging other people’s lives prolonged mine.

If you want to live life to the fullest, not feel you wasted this gift, filling the heart is what life should be about; To serve others unconditionally, with all your heart, enrich others’ lives to make your own life worth living.

Baby Boomers Changing The Healthcare Landscape

Kendra and Jay Jeffcoat are the quintessential baby boomer couple.

Kendra, 69, recently retired after a long and successful career as a college professor and administrator.

Jay, 70, who spent his college years as student body president at the University of California, Santa Barbara, protesting the Vietnam War, continues to work full time as a corporate attorney in San Diego.

In January 2013, Kendra was diagnosed with stage 4 lung cancer, which had metastasized in her brain. She never smoked, nor was she around secondhand smoke.

But as Healthline noted in November 2014, lung cancer among women who don’t smoke is on the rise.

Kendra endured brain surgery, several courses of chemotherapy, and a clinical trial for a cancer vaccine.

She is now being treated with Opdivo, a new immunotherapy medication from Bristol-Myers Squibb that helps a person’s immune system fight cancer. It was approved last year by the Food and Drug Administration (FDA).

The emergence of immunotherapies is considered a game changer in the treatment of cancer. Kendra and her husband are pleased that this new treatment is available.

Kendra said she was shocked when she was first diagnosed, but age has given her the wisdom to deal with it.

“The joy of being over 60 is that a person has the experience and wisdom to deal with the realities of a crisis with joy, hope, and laughter,” she told Healthline. “Each round of chemo, radiation, vaccine therapy, and now immunotherapy, provides an opportunity for a roller-coaster ride from sadness to hope. You must be strong and surround yourself with optimistic people who love you deeply enough to tell you the truth. Being a baby boomer, I can see the ironies and humor of life, and thrive joyfully in the midst of whatever challenge I meet.”

Added Jay, “We have a great life. We’re both very active, I exercise every day. We spent three weeks in Ireland last year. We’ve been to South America. And we hope to take a river cruise in St. Petersburg, Russia, this year. These new treatments hopefully will allow us continue enjoying our life.”

Baby boomers turning 70

The oldest members of the baby boom generation turned 70 this year.

But this group of nearly 75 million men and women born between 1946 and 1964 still see themselves as young and essential.

The boomers are determined to stay young, hip, and healthy.

Today, the generation that preached “don’t trust anyone over 30″ is heeding Dylan Thomas’ call to “not go gentle into that good night.”

They’re stubbornly raging against the dying of the light as they set out to prove they aren’t done just yet.

As a result, this generation is having a profound influence on healthcare in the United States with a unique set of demands, and challenges.

Boomers are the creators, as well as the recipients, of massive innovations and breakthroughs on multiple health fronts, from new, less toxic cancer treatments to an all new paradigm for American emergency medicine.

Overall health concerns

Baby boomers dominated the American cultural landscape for decades.
They don’t appear too eager to relinquish that crown.

But they’re clinging to a greased rope.

Popular culture and social media are now largely shaped and defined by millennials, the young men and women, ages 18 to 35, who now number 75 million.

Generation X, which is made up of people aged 36 to 51, is also projected to pass the boomers in population by 2028, according to the Pew Research Center.

Although there are plenty of misconceptions about boomers, perhaps the biggest one is the notion that they are healthier than previous generations.

In fact, a new United Health Foundation study of the health of senior Americans found that boomers will be entering their senior years with “higher rates of obesity and diabetes and lower rates of very good or excellent health status, putting significant strain on the healthcare system.”

In fact, the Trust for America’s Health recently noted that 62 percent of Americans between the ages of 50 and 64 currently have at least one chronic condition, such as heart disease, because of obesity.

In an interview with Healthline, Dr. Dilip Jeste, professor of geriatric psychiatry and director of the University of California, San Diego, Center for Healthy Aging, explained why boomers aren’t quite as healthy as one might think.

“With many jobs requiring more sedentary work, longer commuting, more television watching, etc., middle-aged baby boomers were found to have less average physical activity than middle-aged folks from previous generations,” said Jeste.

“While smoking has become less common, physical activity has not increased,” he added. “On top of that, sodas, sugar-rich juices, and double cheese burgers are making the diet more unhealthy.”

Jeste said boomers also have higher rates of depression, anxiety disorders, and substance abuse than the previous generation. The precise reasons for this are not known, Jeste said, but there are several possible explanations.

“Baby boomers are more open to diagnoses of mental illnesses than the previous generations, and are more likely to seek treatment for them,” he said. “Talking about their own depression or taking Prozac or Zoloft is not as much of a stigma for them as it was for previous generations.”

Jeste said that primary care clinicians are also more open to diagnosing and treating these mental illnesses now than they were 30 years ago.

“There is growing social anomie, with globalization, mobility, increased competition for jobs, higher divorce rates, more single-parent families, and rapid changes with technology, resulting in greater stress,” he said.

But Jeste added that boomers overall are optimistic and determined to stay young and live longer, and that for the most part this is a healthy way of looking at life.

“The main thing is that aging is not just physical. There is, of course, a psychosocial element,” he said. “We are all in part responsible for our own aging. Our center looks at aging in a positive sense. We try to help people enjoy the process. People do become more accepting as they age.”

Breakthroughs in cancer treatment

When it comes to cancer treatments, baby boomers like Kendra Jeffcoat are at the “leading edge” of the precision or customized medicine paradigm, said Dr. Razelle Kurzrock, director of the Center for Personalized Cancer Therapy at the UC San Diego Moores Cancer Center.

“This model will see the introduction of a new way of doing clinical trials,” Kurzrock said. “This concept is applicable to all our clinical trials, since genomics tells us that each patient and tumor is unique. Boomers will see this new paradigm enacted and should be at the forefront of pushing it forward.”

Dr. Sandip Patel, assistant director of the Clinical Trials Program at Moores, added that baby boomers are a key patient group in the development of novel therapies that not only work better against cancer, but are substantially less toxic than traditional chemotherapy.

“These include immunotherapies and targeted therapies in particular, with a focus on developing approaches to personalize therapy based on each individual’s unique cancer,” Patel said. “A large majority of our cancer clinical trial patients at UCSD are in the baby boomer generation.”

Patel said the hospital provides multidisciplinary care “to best help baby boomers fight their cancer on all fronts: supportive and palliative care, psychosocial support groups, as well as some holistic medicine approaches such as acupuncture.

This new wave of innovation in cancer treatment, Pavel noted, “is happening just as baby boomers are approaching the age in which they may need these novel therapies.”

Dr. Ezra Cohen, associate director for translational science at Moores, said it is “not unrealistic to think that the baby boomers will be the generation that experiences the most substantial transformation in treatment ever, with even cures where we never imagined.”

But Cohen added, “They are also the generation that has to help make this happen, and they can by participating in research, lobbying public funding sources, and providing support. It is up to all of us to end cancer.”

Boomer emergency rooms

Another burgeoning new trend in healthcare is the advent of geriatric emergency departments geared specifically to boomers and other seniors.

UC San Diego Health, in a partnership with West Health, a nonprofit health organization, is currently designing a state-of-the-art senior emergency care unit to be housed within the future Jacobs Medical Center, a 10-story, expansion of UC San Diego Health’s La Jolla campus that opens later this year.

Largely answering to the fact more than 10,000 baby boomers in the United States turn 65 years of age each day, the new emergency department will focus on geriatric medicine, acute care screening, urgent care, case management, and social and psychiatric care.

With a focus on fostering “successful aging,” the department will also facilitate home- and community-based care options when possible.

“We are delighted to partner with UC San Diego, a leader in aging initiatives, to create an innovative environment for delivering outcomes-based senior emergency care,” said Shelley Lyford, president and chief executive officer of the West Foundation and the West Health Institute. “This model will serve as a catalyst for broader adoption of improved senior emergency care here and across the nation.”

Dr. Vaishal Tolia, an assistant clinical professor at UC San Diego Health whose primary specialty is emergency medicine, told Healthlne that emergency departments are “re-examining the role we play.”

“We are no longer isolated as a separate section,” he said. “We see ourselves now as the central gatekeepers as far as who gets hospitalized, and as a referral point and safety net for all kinds of patients.”

Tolia said his department visited several of the geriatric emergency departments around the country to try to learn best practices.

“We learned that there is no standard to call yourself a geriatric emergency department, there is a lot of variability in places that call themselves geriatric ED’s,” he said. “There are some hospitals that call themselves geriatric EDs, but they do not have a dedicated space or staff. They maybe have a couple of additional things for that patient population but no real established criteria.”

Tolia said a new focus on geriatric emergency departments is improving the overall experience for the patient.

“No one wants to be there and we are want to create a positive, comfortable environment where family can be involved, where we can assist folks that have vision or hearing issues, where patients are safe,” he said. “Just as there are architectural features in pediatric departments, where you see colors, cartoons, things that make it comfortable for a child, we are doing some of the same things with geriatric ED’s including making sure the floors are not slippery and that the acoustic and sound are good.”

In response to this booming trend, the American College of Emergency Physicians (ACEP), American Geriatrics Society (AGS), Emergency Nurses Association (ENA), and Society for Academic Emergency Medicine (SAEM), recently issued a comprehensive set of guidelines covering everything from staffing to education to handling common problems of aging, such as falls, delirium, and dementia.

“Geriatric emergency departments first appeared in 2008, but this is the first time there has been a standardized template for how they should be set up and how care for older patients should be delivered,” Alex Rosenau, president of ACEP, said in a statement. “It is important that the special needs of these vulnerable patients are met appropriately in the emergency setting. As of 2010, there were 40 million people in this age group, and many of them will be emergency patients at some point.”

Breakthroughs in dementia prevention

Other issues that are on the minds of baby boomers as they get older are Alzheimer’s disease and dementia.

There are significant new clinical trial results in Alzheimer’s, the most common form of dementia.

While scientists from the Netherlands have found that a six-year, nurse-led vascular care intervention did not lead to a reduction of all-cause dementia in a cognitively healthy population, fewer cases of non-Alzheimer’s dementia were observed in the intervention group compared to the control group.

In addition, the scientists saw fewer cases of incident dementia in a subgroup of people in the study with untreated hypertension who were adherent to the intervention.

The study observations suggest that “the benefits — or the head and the heart– of assessing, treating, and managing heart health risk factors as we age,” Maria C. Carrillo, Ph.D., chief science officer of the Alzheimer’s Association, said in a statement.

An article in May 2015 in Alzheimer’s & Dementia: The Journal of the Alzheimer’s Association, noted that “there is sufficiently strong evidence, from a population-based perspective, to conclude that regular physical activity and management of cardiovascular risk factors (diabetes, obesity, smoking, and hypertension) reduce the risk of cognitive decline and may reduce the risk of dementia.”

Desert trip a bad trip?

Perhaps the final big national bash for the baby boom culture is the upcoming Desert Trip, a megaconcert in the Southern California desert featuring six of the most iconic musical acts and artists in the baby boom canon.

That would be Paul McCartney, Bob Dylan, the Rolling Stones, the Who, Neil Young, and Roger Waters of Pink Floyd.

Predictably, the jokes are already flying about this concert, which folks on social media are calling “Oldchella” or “AgeCoach.”

Among the quips:

“They’ll be trading selfie sticks for walking sticks.”

“Instead of acid, the drug of choice at Desert Trip will be antacid.”

“Has anyone called the Hells Angels to police Desert Trip?”

Desert Trip takes place over two weekends in October at Empire Polo Club, the site of Coachella.
But unlike Woodstock, one of the boomers’ defining events, this one is unlikely to see much mud or protests.

The event, which is already sold out, is expensive and not for those boomers on fixed incomes.

The demographics of the Desert Trip audience will likely cut across four generations. But there will undoubtedly be many boomers in attendance, which poses some interesting health and safety challenges for planners.

Could some aging boomers attempt to relive their Woodstock or Monterey Pop Festival days and take drugs and dance all night?

When asked what precautions the concert planners are taking in terms of providing doctors, nurses, first aid stations, and more at the concert site, a spokesman for the Desert Trip declined to comment.

In other words: Boomers, if you’re headed for Desert Trip, take good care, because apparently you’re on your own.
